There are several ways to complete the USARM application/renewal form. You can download a MS Word document version of the application, or a PDF (Adobe) version or print a text version.

The choice is yours. If you need this application in another format, please send an email to the USARM Webmaster, and we'll see what can be done.

If you do not have Microsoft Office to open the DOC file you can download Open Office which is open-source software which will allow you to open and print the DOC files. You will need Adobe Acrobat to be able to print the PDF files. You can download Adobe Acrobat Reader.
